The Indianapolis Coalition for Patient Safety was formed in 2003 to provide a forum for Indianapolis-area hospitals to share information about 'best practices' and work together to solve the most concerning patient safety issues in the Indianapolis hospitals in a non-punitive setting.

ICPS is a non-profit, public/private partnership of health care organization, educators, medical groups and health insurance companies. It is comprised of chief executive, medical and nursing officers from Clarian Health, Community Health Network, Richard L. Roudebush VA Medical Center, St. Francis Hospitals and Health Centers , St. Vincent Health, and Wishard Health Services. Coalition membership recently expanded to include the Suburban Health Organization hospitals ( Hancock Regional Hospital , Hendricks Regional Health, Henry County Hospital , Morgan Hospital and Medical Center , Riverview Hospital , Witham Health Services and Westview Medical Campus). In addition, there is participation by entities such as Eli Lilly,WellPoint, Inc, Indiana and Purdue Universities , and the Regenstrief Institute, Inc.

The ICPS was formed, in part, as a response to a report issued in 1999 by the Institute of Medicine. In that report, "To Err is Human", it was estimated that 44,000 to 98,000 patients die every year as a result of health care error. This report lays out a comprehensive strategy by which government, health care providers, industry, and consumers can reduce preventable medical errors.
