The Indianapolis Coalition for Patient Safety ("ICPS") is committed to protecting your privacy. This
Online Privacy Policy will provide information to you regarding how we collect and use your personal
information, and about choices you have concerning our use of such information. Please read this Online
Privacy Policy carefully. From time to time, we may revise this Online Privacy Policy to address new
issues or changes to our web site or practices. These changes will be posted on this page so that you

This Online Privacy Policy applies to ICPS 's method of collection and use of information gathered by us
through your use of http://www.indypatientsafety.org.
The privacy practices of third party sites are not covered by this Online Privacy Policy. Once you leave
this web site via a link or otherwise, this Online Privacy Policy does not apply and ICPS cannot and is
not responsible for how data and information obtained from you on other web sites is used by the
operators of such web sites. ICPS disclaims any liability for any third party's use of your Personal
Information obtained through using the third party web site.
Types of Information Collected by ICPS

title, (iii) address, (iv) email address, (v) telephone number, (vi) password, (vii) credentials
and (viii) affiliation.
Personal Information is collected in order to provide superior service to you. We may also request
Personal Information from you in connection with your use of, or participation in, the web site,
including registrations, requests for additional information or in connection with other services or

ICPS does not currently collect cookies (small computer files that are transferred to a computer's hard
drive and which provide knowledge about site visits).
Privacy of Children
We are committed to protecting the privacy of children. The ICPS web site is not intended to or
designed to attract children under the age of 13. We do not collect Personal Information from any
person we actually know is a child under the age of 13.
Personal Health Information
The ICPS web site is not intended to or designed to solicit personal health information. ICPS does
not collect personal health information via the ICPS web site.
Security
We believe that protecting the security of your Personal Information is critical. Therefore, we have
put strong security measures in place to protect your Personal Information. Access to Personal
Information is limited to ICPS employees or other authorized individuals, and identities are
verified by internal network access rules and application username/password pairs. Data is physically
stored through a third party data system at private facilities on servers leased by us. Data fields
that identify individuals are separated from non-sensitive data fields. Data identifying Personal
Information is encrypted through use of industry standard SSL 128 bit encryption methods and practices.
Data decryption keys are kept physically separate from encryption keys and encrypted data. Records are
marked with tamper-proof authentication digests.
Access to Personal Information
At your request, we will use reasonable efforts to provide you with access to your Personal Information
and to revise, update or replace Personal Information that you inform us is in error. Please contact
